OSHA issues Fines after Maui Zip-Line Employee's Death

After investigators concluded that a new zip-line employee's fatal fall resulted from inadequate safety measures, the Occupational Safety and Health Administration issued $7,000 fines to both Altres Inc. and owner Jeff Baldwin of Baldwin Brothers LLC.

 

On the morning of the employee's death, she was pulled off the platform where she was assigned to capture a zip-line customer coming from another platform.  The employee was not wearing a properly latched safety harness when she was pulled off the platform, and the company's policy manual made it optional to wear the harness.  After being pulled off the platform, the employee gripped onto the customer as they hung over the ravine, before losing her grip and falling to her death.  A coworker who caught another customer around the same time was properly strapped into a harness and survived.
