The Department of Labor has issued revised model Family Medical Leave Act (FMLA) forms. The forms were revised to add language instructing health care providers not to provide information on genetic tests, genetic services, or the manifestation of disease or disorder in the employee's family members. The new forms expire on May 31, 2018 and should be used by employers immediately.
